LSSURP online application instructions
Please make sure to review all the items below carefully to submit your completed application.
- Create user name and password (remember for future login).
When you have successfully created your user name and password,
it will be indicated by “saved data” in red print. Then
press the “Continue to Application” button to proceed
to the application.
- Fill out application completely, make sure to press save
changes after each section (this is not a requirement, just
in case you run into computer problems). There is a timed
out feature of one hour if there is no activity, therefore
you may wish to print a copy of the application so you can
gather the information that will be needed before starting.
- Prepare Personal Statement & Biographical Sketch to Upload
(see instructions while logged in). Press “Choose file”;
select file from your computer; once selected and uploaded
press the “save changes” button. You will then see “Your
personal narrative has been successfully loaded”. Remember
to do this for both documents.
- Once you have filled out all the required fields in the
application and uploaded the 2 documents, check the box
in the light grey shaded box
and press the “save changes” button to submit your completed
application. If you receive “Missing fields! Submission of this application has been rejected! Please answer all the required fields.” message please go back through the application
to check what is missing. Once you fill in the missing fields
you will have to re-check the box in the light grey shaded area and
press the “save changes” button again to re-submit your
application. When you have saved your changes and submitted
your application, you will see “Congratulations! Application submission accepted.” Then press the “logout” button to log out.
- Print letter of recommendation instructions (PDF) and give to two (2) faculty members to fill out and return to our office electronically.
- Request Official transcript to be sent to our office
via US Mail. Also, submit one unofficial transcript (it
can be a copy) and send to our office electronically
or via US Mail. Make sure to include courses in progress for Spring 2010.
